Generations Health Care Initiatives
The Generations Health Care Initiatives has formulated the Bridge to Health Survey which is distributed throughout NE MN and NW WI to obtain valued information on resident health status. This survey has been a collaborative effort involving organizations that represent public health. The survey is designed to gather population-based health data on adult residents in 8 MN counties and Douglas WI. The survey is distributed randomly to a selection of adult residents in the selected counties. The topics of the survey include; health conditions, lifestyle factors, utilization of health screenings, access to physical, mental, and dental health care.
The purpose of this survey is to provide information that will enable organizations and communities to better understand the health of their community and guide any intervention strategies.
